The Coordinated Entry Case Conferencing: Youth meeting occurs monthly on the first Wednesday at 2-3pm. The session is facilitated and the meeting invite is managed by Belle Homer (CE Lead) for the H2H CE program.
Topic of Meeting: The Youth session is meant to have service providers review the list of participants on the H2H CE PL Report aged 18-24 who have requested assistance so referrals can be made for clients to connect with a provider, and any updates to client cases can be recorded. Program information and updates may be shared but is not the intended focus of this session.
Who should attend: Service provider staff who work directly with clients to provide case management, housing navigation, resource navigation, or other supportive services.
Providers who are currently represented include the HOTBHN YHDP team, HOTBHN DOBEY team, HOTBHN Klaras Center for Families, Lifeworks Austin/ WHA, WISD HMSS, the Cove, MCH.
